Event Request Form

Welcome to the FBC Online Event Request form. Please take a few minutes to read the page in its entirety before beginning the form. If you have any questions, please feel free to call the church office.

What to do…..

  • Complete the FBCTT Event request form 4 weeks prior to the event date (If the event is less than 4 weeks out, the event runs a high risk of being rejected.)
  • The form can be obtained from the church office, filled out online
    or downloaded below.
  • Return the form to the church office. (if not submitting online)
  • The event will be reviewed for approval and the event contact person will be notified within 3 business days regarding the event status.
  • If you are requesting our facilities for a wedding, you must work through our wedding coordinator, Robin Lowery. NO FORMS will be accepted for weddings unless turned in by her. You may download the wedding brochure here.

Please note, once an event has been approved, any subsequent changes (changes in time, number of attendees, cancellations, etc.) must be communicated to the office as soon as possible and no later than 1 week prior to the date of the event via email here (Louise Vetzel) or 988-1138. If changes are received less than 1 week from the event, you will become responsible for both making the changes yourself, as well as putting the changes back to the original request.
